2. You need to have the following:
 
  • Printer
  • Voided Check
  • Annuity Training Certificate
  • Names of companies with which you are currently appointed
  • E & O Certificate
  • LTCI Continuing Education Certificate
  • Tax ID Number
3. If you need an Errors & Omissions insurance certificate, just click HERE. We provide for you, at no charge to you, $1,000,000 E&O for business through Art Jetter & Company.
4. If you are completing this for someone else, enter only information for the person being appointed. Do not use your own information.
